I have concerns about my child, what do I do next?

• If your child is less than 2 years, 9 months old:

  • Call the Connecticut Birth to Three system at 1-800-505-7000, or 211 and follow the prompts.

  • Complete the Sparkler questionnaire 

    • Download the app Sparkler

    • Use the code Middletown when prompted

 

• If your child is 2 years, 9 months or older:

  • Call the Middletown Preschool Program for a screening

  • Complete the Sparkler questionnaire 

    • Download the app Sparkler

    • Use the code Middletown when prompted
